Before scheduling your first visit, you must set up your patient portal and complete all required intake forms so we can review your medical history and process your information correctly.
PLEASE NOTE:
You must create a patient portal account first before completing the intake forms. If the portal account is not created successfully, paperwork may not transmit correctly.
Email admin@ptpmn.org with your full name and birthdate to set up your patient portal and begin your intake
After sending your request, our admin will set up a Patient Portal for you and send instructions for logging in and completing all necessary intake forms.​
