Any paperwork requests—including but not limited to prior authorizations for medications, letters of support for emotional support animals, requests for leave of absence from work (such as Family and Medical Leave Act paperwork), disability forms, or other administrative documentation—require a scheduled clinic visit.
Completing paperwork typically takes 20–30 minutes or more of clinical time. This work is not reimbursed unless it is completed during a scheduled visit. For this reason, it is important that you are present so we can review the information together, ensure accuracy, and complete the documentation appropriately.
Paperwork will not be completed outside of a visit, through portal messages, or retroactively without an appointment.
