Set Up Your Patient Portal

NEW PATIENT INTAKE

Before scheduling your first visit, you must set up your patient portal and complete all required intake forms so we can review your medical history and process your information correctly.

Important: Forms Must Be Completed Before Scheduling. Your appointment cannot be scheduled until all 7 forms/steps are completed.

Email admin@ptpmn.org with your full name and birthdate to set up your patient portal and begin your intake

After sending your request, our admin will set up a Patient Portal for you and send instructions for logging in and completing all necessary intake forms.​

Why am I being redirected?
Miles Med Management works with Psychotherapy Partners, a group practice that offers professional and secure intake, billing, and administrative support for all of its practitioners and clients. You may notice their website during this process — that’s completely normal and part of your confidential setup.

AFTER YOU  RECEIVE VERIFICATION THAT YOUR PORTAL IS COMPLETE: SELF SCHEDULE

PLEASE NOTE: Your appointment will not be valid until all 7 FORMS are processed and admin has sent you a confirmation. If these steps are not completed within 24 hours of your appointment, then your appointment will be canceled. 

You will need to upload your insurance and credit card information (for co-pays) and fill out the necessary forms for insurance billing. Then you can access our convenient self-scheduling feature. Please allow the billing department up to five business days to ensure that all paperwork is thoroughly processed before scheduling your new visit.​
