EXISTING PATIENTS

Your insurance information and balance must be up to date before scheduling your appointment. If you have an outstanding balance or your insurance is out of date your appointment could be subject to cancellation.

NEW PATIENTS

Your appointment will only be valid if you have completed all 7 intake forms. If these 7 steps are not completed by the day of your appointment then your appointment will be canceled.

You will need a 60-minute time slot for your intake appointment. Because the scheduling app only allows 30-minute sessions, please book two back-to-back 30-minute appointments to create the full 60-minute intake slot.

Please schedule your appointment at least five business days after you have completed all intake paperwork. This allows the business office enough time to review and process your information before your visit.
